Henkilötunnus – Social Security Number. If you dont have finnish Social Security Number, write your birthday in form (if you have born for example first december 1.12.1970): 011270- and if you have born 2000-century (for example first january 1.1.2000): 010100A

Työtön= Unemployed, Työssä = I am at work, Opiskelee = I am studying, Muu = Other (You can write to empty line, but not necessary), Eläkkeellä = Retired

Laskutusosoite, jos eri kuin yllä = Invoicing address, if payer of your cost is other than yourself

Accommodation? = Cross, I need accommodation during workshop

Majoituksen lisätiedot: Päivämäärät ja huonekaveri = Dates you need accommodation, single room or double room and who you want to be with in the same room (Single room is 140 euros more expensive!)

Information of accomodation hotel, where workshop is. Place is also christian education center and there is parlament of lutheran church gathering two times a year.

I linken finns svenska presentation av konference hotell, där workshop ska hållas. Platsen är också Kristliga institutet för vuxna och Lutherska kyrkans kyrkomöte samlas där två gånger i året.
